Georgia Resident — Must currently live in  Georgia

Medicaid-Eligible — Must be enrolled in Medicaid or currently applying

Needs Assistance — Loved One Must require help with daily living activities

Family CaregiverFamily member or trusted caregiver available

Eligibility is confirmed during intake with a care specialist.
